HSI uses a comprehensive multi-tiered approach that engages prevention, intervention, treatment, research, and care management to build healthy communities.

HSI Vision

HSI through a continuum of care approach is building healthy communities for underserved populations with disabilities and people living with an array of human services needs.

HSI’s Land Development Corporation was created for the purpose of constructing and rehabbing affordable housing for seniors, people with disabilities,veterans and persons with low to no income; to ensure a greater degree of Neighborhood Stabilization. When matched with our janitorial program, these persons can benefit from work, housing and social services support.

These Habilitative Systems, Inc. HUD 202 Apartment Communities are managed by Prairie Management. Each facility provides 59 one-bedroom unfurnished apartments with a large living room/dining area combination, full size kitchen that includes stove and refrigerator.

Our senior living program includes a Service Coordinator for the residents. Service Coordinators helps the tenant obtain services that they need to maintain independently living with dignity.

HSI constructed five 811 houses for persons with disabilities on the West side of Chicago. These units provide additional housing for this population and allow for placement in both a community based and lesser restrictive environment.
