Job Summary:

Complete initial review of persons referred from DMH Determine in the resident review if the mental health needs, identified in the PAS level II assessment, are integrated into the individual's facility care plan.  Offer the resident(s) choices in living arrangement based on treatment issues supported by clinical assessment. Determine if the individual is ready to transition to a less institutional setting or if the resident is in need of a greater level of care based on their functional abilities and thus clinical need. Refer residents eligible for nursing care on a time limited basis for targeted case management.

Qualifications:

Bachelor's Degree preferably in Social Services with a minimum of two (2) years work experience in the

social service field in a nursing home environment is preferred. Must be computer literate and demonstrate

good oral and written communication skills.
