HSI is among the largest human service employers on Chicago’s Westside and operates 39 programs in 15 Westside locations. HSI’s 200+ employees, interns and volunteers serve well over 7,000 people at risk of institutionalization, hospitalization, incarceration and mortality annually; thereby reducing high institutional and emergency room care costs significantly.

Provides residential housing in a community setting and an array of responsive services for 8 homeless adult mentally ill consumers with or without a co-occurring disorder of substance abuse and will help them achieve their highest level of self efficiency.

Williams Consent Decree Community Supervised Residential

Community Supervised Residential Services For Williams Consent Decree Class Members: Provides transitional residential and support services to  individuals diagnosed with serious mental illness, who are residents of nursing facilities as they transition to permanent  supportive housing in accordance with the Williams Consent Decree.  These individually based services include a community support team, case management, life skills and counselling.

The Child & Youth Services Annex serves urban children, youth and families in need of support and intervention to remain intact, become independent and/or reach their potential.

The Veterans Center provides services to those at risk or suffering from: Post-Traumatic Stress Disorder ● Homelessness ● Un-employment/Under-Employment ● Civilian Reintegration ● Suicidal Ideation ● Alcohol/Substance Abuse ● Domestic Violence  ● Military Sexual Trauma

 One of four newly constructed 811 houses for persons with disabilities. These units provide additional housing for this population and allow for placement in both a community based and lesser restrictive environment. The project is funded by the U.S. Department of Housing and Urban Development

 Provides supervised residential homelike environment for persons in crisis situations who suffer psychiatric illness and/or substance abuse. Services are available to persons who are unable to live independently, but do not require hospitalization.

These Habilitative Systems, Inc. HUD 202 Apartment Communities are managed by Prairie Management. Each facility provides 59 one-bedroom unfurnished apartments with a large living room/dining area combination, full size kitchen that includes stove and refrigerator.

Our senior living program includes a Service Coordinator for the residents. Service Coordinators helps the tenant obtain services that they need to maintain independently living with dignity.

1st Female Veterans Facility in Illinois for Treatment of Substance Abuse. A 28 bed recovery home for chemically dependent women who are in the recovery process. Women in this residential program receive individual and group counseling and intensive case management services to develop and implement strategies to meet the objectives of independent and drug-free living. More permanent housing and employment are primary focuses while in the program.

Urban Behavioral Health Services Institute               

This outpatient mental health center will offer an array of services to adults 18 years and older. 


Services include:

  • Individual, Group and Family Counseling
  • Case Management Services
  • Crisis Intervention
  • Psychiatric  Evaluation
  • Medication Management
